## Admissions &amp; Assessment

##  Active Treatment





You’ll participate in programming designed to support both healing and belonging. Treatment may include individual and group therapy, nutritional support, meal preparation assistance, and creative or movement-based activities. Care is provided in inclusive, identity-affirming spaces where you are respected and understood.

## Step-Down &amp; Aftercare





As you make progress, we’ll guide you in stepping down to IOP or outpatient therapy. Discharge planning begins early and may include relapse-prevention strategies, follow-up appointments, peer support groups, and community resources to help you maintain your recovery in the long term.



















## What happens after treatment?



Treatment doesn’t stop at discharge. As you transition to outpatient or community-based care, our team will work with you to design a structured aftercare plan that supports long-term recovery. This plan may include evidence-based therapy, nutrition counseling, medication management, or peer support services, each tailored to your needs and goals.

Aftercare planning begins early to ensure continuity of care. We emphasize relapse-prevention strategies, scheduled follow-up appointments, and access to peer and community resources, so you have practical tools to stay on track. Our priority is to help you feel both prepared and supported as you carry recovery into daily life. You will never have to walk this journey on your own.
